I have used US Bank for the past 8 years and have their Visa Debit Card.  I never leave home without it. : )  In all the time I have been with this bank and used this card, I have never had a problem of any kind.  A few times when there were "questionable" charges on my statement, they immediately investigated and removed them for me.  Their customer service has always been exceptional, whether by phone or in person.  I would not even consider using any other bank or debit card.  They are the best!
