I got sick of high prices and poor service, and switched to Vonage. The sound quality is not always great, but it seems to be improving. I love just about everything else about Vonage. It's cheap, I'm not paying money to the big companies, I can make unlimited local and long distance calling (please note: I don't generally call outside the US -- I know you can make out of country calls to some countries, but you should check it out and make sure you can make the calls you need).
It's also got lots of features for free that you'd pay more for with the big phone companies -- call waiting, caller id, 3-way calling, voicemail (which you can access on your phone, through another phone, or online), call forwarding (and you can give them a number to automatically forward your calls to if your broadband goes out), etc. etc. etc. If you travel a lot, as long as you have broadband where you're going, you can take the Vonage adapter with you and get your phone calls wherever you are.
Since you can have any area code you want regardless of your actual location (including toll-free numbers), you have to remember to register your address so that if you dial 911 the responders know where to go. And if you take the Vonage adapter with you somewhere, you have to remember that if you dial 911 they're going to go to the registered address, not where you actually are.
