Electric mattress pads are fantastic!  I first encountered them in Scotland at a B & B.  It is placed between the fitted sheet and the mattress so the heat radiates upward and is trapped around your body by the blankets.  Many now have, in addition to heat regulators, the option of heating the foot section a bit more than the rest.  The queen sized and up have dual heating controls for you and a partner.
