It is very impressive in containment of smell depending on the size of the room you put it in. First it was in our daughter's room (~90 square feet) and when we used to open the door of her room after about 3-4 hours, we could feel the smell of diapers. We moved it to the adjacent (much bigger) room and we don't feel the smell anymore (it must still be there, only diluted). And when we open its lid to put a new one, it does smell pretty bad. But considering that smell and how we don't smell a thing when the lid's closed, it delivers a pretty impressive performance.
When it comes to refills, well there are (almost) always a catch in good things, right? And by now, we should all be aware that refills are how companies get you in products like these :) So we should know what we are in for and still be interested to buy in products that need sustainable use via the presence of an attachment/refill. For this particular case, I think this con is well-compensated by the pro though. I'd definitely recommend it.
